WHAT IS COCCYDYNIA?

Coccydynia is pain and discomfort in the tail bone region. Coccyx is made up of 3 to 5 fused vertebrae and it represents the vestigial tail. It is more common in ladies. The usual reasons for coccyx pain could be sudden fall with impact on the tail bone, repetitive trauma or child birth.

WHAT CAUSES TAIL BONE PAIN?

Repetitive trauma this can lead to dislocation of the coccyx bone which results in pain. Activities like riding bicycle or 2 wheelers can lead on to coccyx pain. The pain is worse while sitting for long duration and during bowel movement. Coccyx pain can often occur without fracture or dislocation.

HOW IS COCCYDYNIA TREATED?

a) Conservative management may include mild pain killers or a short course of NSAIDS or mild opioids. Physical therapy and exercises like pelvic floor strengthening and groin stretches may relieve coccyx pain. Avoiding repeated trauma like 2 wheelers / cycling, may help. Coccyx cushions are doughnut shaped cushions and they ensure minimum pressure on the coccyx will help in healing. Methods / medication to relieve constipation, if present, will help. Manipulation of coccyx may help in dislocation injuries.

b) Your pain physician will decide which treatment will suit you best. If conservative management fails, he may offer.

  • Pericoccygeal steroid injection/ coccygeal nerve RF ablation, under x-ray guidance. This day care procedure reduces inflammation and pain arising from coccyx.
  • Ganglion impar block may be useful in patients with serve coccyx pain. This is an x-ray guided, day care procedure.

In patients refractory to all treatment, very rarely, surgical removal of coccyx is performed.
